On your first visit you will need to fill out a client history form and possibly a COVID-19 waiver form as needed. The forms are below that you may print and fill out prior to your visit to save us both time, or I will have them in office for you.
When you first arrive, if the door is closed with "Do Not Disturb" sign on it, then I may be in with a client still or cleaning up the room. Please have a seat in the waiting area, I will be out momentarily.
